ACR AND EULAR RECOMMEND TARGETING DORIS REMISSION AND PREVENTION OF ORGAN DAMAGE1,2

The 2025 American College of Rheumatology (ACR) Guideline and the 2023 European Alliance of Associations for Rheumatology (EULAR) have updated their recommendations for the management of systemic lupus erythematosus (SLE) to target DORIS remission as part of a treat-to-target strategy (or Lupus Low Disease Activity State [LLDAS] if remission is not possible).1,2

DORIS REMISSION COMPONENTS*,2,3

MINIMAL STEROID USE

Prednisone/equivalent dose ≤5 mg/day

NO CLINICAL DISEASE ACTIVITY

Clinical SLEDAI=0 and PGA <0.5 (0-3 scale)

DORIS allows for antimalarials and/or stable immunosuppressants, including biologics.2,3

ACR and EULAR SLE treatment goals acknowledge DORIS remission as a validated and widely accepted clinical treatment goal1,2

SELECT KEY POINTS1,2

Silhouette with Star

TREAT TO DORIS REMISSION

ACR and EULAR recognize DORIS remission as a validated, preferred treatment target*

USE BIOLOGICS EARLY

Initiate treatment promptly, with universal use of HCQ and early introduction of biologic and/or conventional IS therapies

Less Steroid Pills

MINIMIZE STEROID USE

Taper steroids to a dose of ≤5 mg/day and withdraw entirely when possible

*Definition Of Remission In SLE (DORIS): clinical SLEDAI=0, PGA <0.5 (scale 0-3), prednisone dose ≤5 mg/day, on antimalarials, and/or stable immunosuppressives, including biologics.2,3

DORIS remission was established and recommended as the single consensus definition of remission in SLE by an international task force in 2021. The 2023 EULAR recommendations and the 2025 ACR SLE Guideline specify that remission should be defined by the DORIS criteria.2,3

2025 ACR SLE Guideline strongly recommends to taper OCS to ≤5 mg/day (and ideally to zero) within 6 months for patients with stable, controlled SLE on OCS >5 mg/day.1
